On the night of November 26, 2022, shelter employees called emergency services after discovering water and smoke coming from Fleming’s apartment at a Bronx family shelter. Responding officers found Fleming acting irrationally and transported her for psychiatric evaluation. Police initially missed the bodies of both boys, which were hidden beneath a pile of clothes in the bathtub. Columbus Canada, Fleming’s then-boyfriend and father of 11-month-old Octavius, discovered both children unresponsive shortly after police departed and immediately called authorities.
